In the health care industry competition does not lower costs. Let's suppose a hospital was the first in their city to get an MRI machine and based on the number of people who will require an MRI per year - say 1000 - they deduce that they will have to charge $500 per scan to pay for the machine. Then when another hospital gets the same machine the price goes up to $1000 per because they will only get half the 1000 patients. Then when four hospitals get an MRI the price must rise by a factor of four.

That is excellent advice and I in no way intend to discredit it, but if you don't have insurance a trip to the emergency room could send you into bankruptcy.

Nope. Your example decribes collusion. Usually in this case, the one will drop their price slightly to entice patients, the next will drop theirs below that and so on until finally you will end up with the patients paying less. And for additional diagnostics that they wouldn't have had had the machine not been brought.

Alternatively, in state run health service, the hospital will pay over the odds for the machine (using taxpayers money), and then only use it between 10h00 and 16h00 (because the unionised staff don't want to work afterhours), so by the time it is obsolete, it has only been used for 1/4 of what it could have been used for.
